K’s Care Plan -
Personalized Chemotherapy Treatment Guide

K's Care Plan condenses over thirty pages of cancer treatment information into five curated pages. The Care Plan acted as a friendly guide for K and her family throughout her chemotherapy treatment.

Situation:

K was diagnosed with cancer. In preparation for chemotherapy, the cancer center presented her with pages and pages of text detailing each drug, its side effects, and ways to minimize those side effects.

Her initial treatment was a combination of two chemo drugs with some side effects and mitigation strategies that conflicted with one another. K read every word of every page and still had lots of questions.

At K's pre-therapy consultation she asked questions and took copious notes. She left with the pages and pages of text she had received now marked with highlighter and handwritten notes. She also had additional notes in a notebook.

K's Care Plan document pares down the mounds of information provided by the cancer center along with K's notes to actionable items, relevant safety measures, side effects to watch for, as well as daily self-care practices to follow during treatment. The information is grouped and presented visually with clear headers. The calendar section informs K’s spouse and children where she is in her treatment routine. Important phone numbers are prominent, and which conditions are considered emergencies are clearly stated.

Outcome

K's family posted the Care Plan in their kitchen where everyone could see and pass by it daily. K appreciated the easy-to-digest presentation of information and friendly colors while her spouse appreciated the clear instructions on when to call the doctor. The entire family referred to the document to help them understand how to be helpful and navigate K’s treatment the best way possible.


Second Care Plan

K progressed onto a second chemotherapy treatment, and another Care Plan document was created. By then, the family was familiar with the general safety measures to take at home (page 2). The other four pages of the document were posted in the kitchen.
